I understand and acknowledge the following (separate checkboxes)
1. Assumption of Risk: I understand that by allowing my child to participate, I assume all risks associated with such activities, including but not limited to muscle strains, sprains, falls, and other potential injuries.

2. Health and Medical Condition: I certify that my child is in good physical condition and has no medical or health-related issues that would prevent them from safely participating in the programme. I acknowledge that it is my responsibility to inform coaches and youth programme coordinator of any relevant medical conditions or limitations.

3. Parental Supervision: I agree to supervise my child outside of the scheduled sessions and understand that XFC is not responsible for the supervision of my child beyond the designated session times.

4. Consent to Treatment: In the event of an injury or medical emergency during XFC activities, I authorize XFC to seek emergency medical treatment for my child, including calling emergency services and administering first aid as deemed necessary.

5. I hereby give permission for X-Roads Warriors FC to use images/videos of my child for promotional materials on Facebook or any other social media Platforms.

6. By submitting this registration form I acknowledge that I have read, understand, and agree to the terms and conditions outlined in this waiver. I understand the risks associated for my child and voluntarily consent to their participation. I also understand that any activity that my child or I take part in as a player, parent, guardian, or volunteer of X-Roads Warriors FC (XFC) is at my own risk. I acknowledge that XFC will be under no liability whatsoever to the undersigned. I waive all claims (including negligence claims) against and indemnify XFC for any injury or loss sustained (whether resulting from negligence or otherwise) as result of my taking part in any activity of XFC. I understand that this waiver and indemnity extends to all members of XFC, and its associated committees, officers, or agents of XFC and that the waiver is governed by Bermuda law and shall bind the successors and assigns of any person signing this waiver.

To ensure a positive and enriching experience for everyone, X-Roads Warriors FC has established clear behavioural guidelines and corresponding disciplinary procedures. Please take a moment to discuss these guidelines with your child before the start of training.

At XFC we expect all players to adhere to the following behavioral guidelines:
1. Respect for Coaches: Listen carefully and respectfully, follow rules and instructions.
2. Respect for Other Players: Bullying, harassment, name-calling, swearing, damaging belongings or any other aggressive behavior will not be tolerated.
3. Respect for XFC Property: Vandalism and intentional damage will not be tolerated.

Players will be informed of these rules, and the consequences for not following them at the first training session. We encourage open communication, and players should feel comfortable talking to a coach or any youth committee member if they encounter discomfort or require assistance.
